The Form Bank is the master catalogue. Adding a form here registers its file and metadata. It does not automatically make the form mandatory for patients.

Register a New Form
First create the actual HTML form inside the /forms
folder, then register its file path here.
How New Forms Are Added
- Create the HTML form. Put the file in
/forms. - Register it in the Form Bank. Use Add New Form.
- Keep it manual unless it belongs in the automatic workflow.
- For an automatic form, update the workflow router separately.
- Test the route using an appropriate patient age/service combination.
